ISLTS provides specialist selective mutism assessment and support for children, young people and adults across the UK.

While selective mutism is often identified in early childhood, it can continue into adolescence and adulthood if left untreated. Our experienced therapists support individuals at every stage of life, tailoring assessment and intervention to the unique needs of each person.

We work nationally with families, schools, colleges, universities and workplaces through in-person and online appointments, helping individuals build confidence and reduce the anxiety associated with speaking.

Selective mutism is an anxiety disorder where a person is unable to speak in certain social situations, despite speaking comfortably in others. Early identification and support are associated with significantly better outcomes. Our assessments are holistic, person-centred, and needs-led, ensuring that we understand the indvividual’s communication within the context of their personality, environment, strengths and vulnerabilities.

Our Selective Mutism Diagnosis Process

Assessment and treatment plans are always tailor-made for every indvividual we work with. 

  1. Free phone consultation
    Our Selective Mutism diagnosis process starts with a free 20 minute telephone consultation with a specialist Speech and Language Therapist. We listen, ask key questions, and advise whether a Selective Mutism assessment pathway maybe appropriate. 
  2. Questionnaires
    If we decide a Selective Mustism assessment pathway is appropriate, we send questionnaires to gather more information. For children and young people, the questionnaire will be sent to parents/carers and to the individual’s educational placement.
  3. Review & discussion
    Your therapist reads and analyses the information, then discusses it with you (and the educational placement where appropriate) to build a clear picture of the individual’s communication profile.
  4. Outcomes & recommendations
    We confirm whether Selective Mutism is the right formulation and provide clear recommendations and suggest the next steps.

Support Options

If we feel Selective Mutism is not the correct diagnosis, we will telephone you to explain our decision and discuss potential next steps, including signposting.

If we do make a Selective Mutism diagnosis, you can choose one of the options below depending on the level of support you need:

Option 1: Support recommendations (no diagnostic report)

  • General support strategies for managing Selective Mutism
  • Signposting

Option 2: Full diagnostic report & therapy recommendations

  • Detailed written report with diagnostic conclusion
  • Formulation of communication profile
  • Recommendations for ongoing support/adjustments at home, work and education
  • Therapy recommendations + general strategies
  • Signposting

Option 3: Full diagnostic report + therapy recommendations + up to 6 hours intervention

  • Everything in Option 2
  • Detailed intervention plan with bespoke targets
  • Therapy resources (where appropriate) + guidance on measuring progress
  • Up to 6 hours used flexibly (e.g. review meetings, liaison, coaching, modelling strategies, and direct work where clinically needed)

How intervention is typically delivered

For many children, the Speech and Language Therapist’s role is primarily consultative – working closely with families, schools and other professionals through multi-agency review meetings, email, phone or Zoom support, monitoring progress and helping to problem-solve challenges as they arise. We maintain a strong focus on practical strategies that can be used consistently across both home and educational settings.

Sometimes, more direct involvement is needed, for example where there are additional speech, language or communication needs, or where therapists need to model approaches such as sliding-in and shaping with parents, carers or education staff.

For teenagers and adults, therapy is typically more functional and goal-led, focusing on real-life situations that matter to the individual. This may include speaking in education, work, social or everyday settings, building confidence around communication, understanding the role of anxiety, and breaking goals into manageable, achievable steps with supportive guidance throughout the process.

Phone Consultation – Initial 20 minute phone call with a specialist Speech & Language Therapist – Free

Not Selective Mutism outcome –  Review of questionnaires and professional reports, feedback with explanation, signposting and next steps – £127.50

All 3 of the below options include – Questionnaires + clinician review:

Option 1 – Support recommendations (no diagnostic report): strategies + signposting – £250

Option 2 – Full diagnostic report + therapy recommendations – £350

Option 3 – Full diagnostic report + therapy recommendations + up to 6 hours intervention – £700

Payment for your selected option is required prior to support/assessment. A payment plan over 3 payments is available for Option 3. In some cases, there may be an additional travel cost (we aim to keep this to a minimum).

Post-diagnostic Support

We offer ongoing support for children, young people and adults with diagnosed or suspected Selective Mutism, including those seeking assessment, workplace support or help managing everyday communication challenges.
